The following member forms are can be downloaded. Larger files may take longer to download, depending on the speed of your internet connection.

Insurance forms

• Insurance Alteration Form - use this form to change your current level of insurance cover 
• Brief Insurance Application form - use this form to apply for Death and TPD cover up to $400,000 and/or Income Protection cover up to $4,000 per month
• Standard Insurance Application form - use this form to apply for Death and TPD cover over $400,000 and/or Income Protection cover over $4,000 per month, or Long-Term Income Protection (to age 65)
• Insurance Transfer Form - use this form to transfer your current insurance with another fund to the Child Care Super
